Federal Loan Forgiveness Program

As a current (or former) employee, you might be eligible for the Federal Loan Forgiveness Program.

1. Our FEIN is 93-6000749.
2. Before submitting the form, please ensure that your social security number is not on the form.  You can fill that in when you receive your completed form. This is for security reasons.  We won't be able to email the form back to you if it contains this private information.
3. Include all pages of your loan forgiveness form.
4. Send the forms to Lisa Spencer / HR Specialist via scan/email at lisa.spencer@nsantiam.k12.or.us or fax to 503-769-3578.

Mortgage or Employment Verification

To verify employment, please send your request to Lisa Spencer in HR.  Requests will be processed in the order in which they are received.
