LOAN FORGIVENESS PROGRAMS (PSLF and TLF)
As a current (or former) employee, you may qualify for the Teacher Loan Forgivess (TLF) or Public Service Loan Forgiveness (PSLF) programs.
- Our Federal Employer Identification Number (FEIN) is 93-6000749.
When submitted your form:
- When using a paper form:
- Do not include your Social Security Number on the form before sending it. You can add it yourself once you completed form back. This is for your security.
- Please include all pages of your loan forgiveness form.
- All requests must be sent to hr@nssd29j.org or (paper) faxed to 503-769-3578.
- If they are being sent through the student aid website, ensure the request is addressed to this email address.

Naming Conventions:
- Computer / Google Sign-in Username: firstname.lastname

INJURED ON THE JOB? (STAFF & VOLUNTEERS)
Learn about worker's compensation and available supports as you return to work/volunteer site.

- Always submit this form to Human Resources (HR) immediately after an accident.
- ONLY SIGN the form if you're going to see a doctor.

MORTGAGE OR EMPLOYMENT VERIFICATION
To request employment verification, please send your request to hr@nssd29j.org. Requests are processed in the order they are received.

- For your first paycheck, or questions about your placement on the salary schedule: contact Dani or Lisa in Human Resources.
- For all subsequent paychecks: Please contact Madeline Block in Payroll at madeline.block@nsantiam.k12.or.us.
ACCESSING STAFF FORMS & SOFTWARE
Intranet: Visit our webpage and select the “For Staff” menu. You'll be prompted to sign in using your firstname.lastname username and the same password as your computer and Google sign-in.
Student Information Services: Please contact your office manager for access.
