FEDERAL LOAN FORGIVENESS PROGRAM

1. Our FEIN is 93-6000749.
2. Before submitting the form, please ensure that your social security number is not on the form.  You can fill that in when you receive your completed form. This is for security reasons.
3. Include all pages of your loan forgiveness form.
4. Send the forms to hr@nssd29j.org or fax them to 503-769-3578.

• Computer / Google sign in - Username: firstname.lastname    
• Email - Username: firstname.lastname@nsantiam.k12.or.us.

INJURED ON THE JOB? (STAFF/VOLUNTEERS)

1. Form for SAIF - Safety Incident Reporting SAIF 801 form 801_SAIF_NSSD_FORM.pdf
   • Always submit the form to Human Resources (HR) right after an accident.
   • ONLY SIGN the form if you're going to the Dr.
   • If you need to see a Dr later, contact HR.

MORTGAGE OR EMPLOYMENT VERIFICATION

To verify employment, please send your request to hr@nssd29j.org.  Requests will be processed in the order in which they are received.

• If this is your first paycheck, or you have a question about your placement on the salary schedule, contact Human Resources - either Dani or Lisa.
• If this is regarding your subsequent paychecks, please contact Madeline Block in Payroll at madeline.block@nsantiam.k12.or.us.

Intranet on our webpage - Select the “For Staff” menu which will prompt you to sign in (firstname.lastname), using the same password as your Computer and Google sign-in.
